We have found that having consistent presence in care facilities helps the patient and staff feel more supported and able to pre-empt care concerns or other issues before they arise.

Weekly wellness visits - $260.00 (3-4hrs)

Frequency visits are tailored to the patients needs based on the services the patient/family requires.
Monthly membershipMonthly Advocacy Membership
$600/month: Four (4) visits per month, 4 phone updates, email and written reports.
$1,200/month: Eight (8) visits per month, coordination with staff, attendance at one care conference, phone updates, e-mail and written reports.
$1,500+/month: Multiple visits each week, rapid response to concerns, extensive coordination, written reports, in-person and e-mail support.
